For New Patients
The first step is to schedule your appointment!
Once an appointment is booked, Dr. Lorn will contact you to fill out the necessary New Patient forms in the Patient Portal. You will receive an email to create an account and start using this convenient portal to access your forms, records, and more.
You must complete all electronic forms 24 hours before your first appointment to ensure Dr. Lorn has adequate time to read your forms in depth. If we do not receive these on time, your appointment may need to be rescheduled. Please take some time to read through the consent forms before your visit, as you will be asked to sign these at your appointment. There is no need to print out any paperwork as the forms will be submitted electronically.
It is important to note that my practice is a green business and is minimizing the use of paper forms as much as possible. However, if you do not have access to the Internet or have trouble creating an account, you are still welcomed to download the following forms to fill out and submit prior to your appointment.
Services & Fees
Initial Consultation Fee (15 minutes): Free
Naturopathic Visits
- First Office Visit*: $325
- Return Office visit 1 hour*: $180
- Return Office visit 30 minutes*: $95
- Metabolism and Immune Injections: $35 per injection
Fees do not include additional procedures, lab tests, or supplements.
Cancellation Fee
A credit card will be required when booking your appointment. If your appointment is cancelled within 24 hours before your scheduled appointment, or you do not show up for an appointment, your credit card will be charged a No Show/Cancellation Fee: $75
